DOCUMENTS REQUIRED
In order to update your name on your membership, documentation is required to essentially connect the old and new names involved in a legal name change. Important note: These documents must be the official certificate, or court order.
A. One (1) of the following government-issued documents that contains both old and new names, and provides an explanation for the change, are acceptable to update the account title:
- Marriage certificate or
- a divorce decree, or
- a court order filed for a name-change proceeding
OR
B. Obtain both of the following documents:
- Current government issued ID with updated name (please refer to our acceptable member identification procedures)
- SSN card with updated name
