    First Entertainment Credit Union Member Story Testimonial Release Form

    I hereby grant First Entertainment Credit Union permission to use my likeness in a member story, which may include text, photograph, video, and/or other digital media in any and all of its publications, including printed publications, multimedia presentations, and web-based publications, without payment or other consideration. I understand my member story testimonial may be used in connection with publicizing and promoting First Entertainment Credit Union. I understand and agree that all stories, photos and videos will become the property of First Entertainment Credit Union and will not be returned. I hereby irrevocably authorize First Entertainment Credit Union to edit, alter, copy, exhibit, publish, or distribute these stories for any lawful purpose. In addition, I waive any right to inspect or approve the finished product, including written copy, wherein my likeness or member story testimonial appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the story. I hereby hold harmless, release, and forever discharge First Entertainment Credit Union from all claims, demands, and causes of action, which I, my heirs, executors, representatives, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization. I understand that this member story will be maintained and made available indefinitely by First Entertainment Credit Union.

    I HAVE READ AND UNDERSTAND THE ABOVE MEMBER STORY RELEASE. I AFFIRM THAT I AM AT LEAST 18 YEARS OF AGE, OR, IF I AM UNDER 18 YEARS OF AGE, I HAVE OBTAINED THE REQUIRED CONSENT OF MY PARENTS/GUARDIANS.
